Business address change:
Form 8822-B is for businesses and other entities with an Employer Identification Number (EIN) application on file. We use Form 8822 to notify the Internal Revenue Service (IRS) of the following changes:
- Business mailing address;
- Business location;
- Identity of your responsible party.
You should mail this form to the following addresses:

- Fill out the 8822-B form, include a copy of the EIN verification letter and write a statement that explains that the CPC-575 letter has the wrong name.

How do I know the IRS had worked on my request:
The IRS will send a 148 A/B notification to inform you this request is already processed which means the information is updated on their records.
Please notice that the new verification letter will be sent to your company's business address.
